Definition of Monthly impressions
When you setup pricing per montly impressions, is that number equal to the number of pageviews in which all ads are shown in one particular page?... or does it refer the the total number of pageviews x the number of ad zones shown in each page?

Good question.
Strictly speaking, "monthly impressions" refers to the total number of ad impressions across all your ad zones in a 30 day period.
So if your site(s) generate 1m page views and each page has 4 ads, then you'll be generating 4m ad impressions.

In addition to this: when there are hundred requests to check if there is an ad available for a certain ad zone, but only 10 requests result in an ad being displayed by Trafficspaces and in 90 cases a fall back script is called to load another advertisement NOT served by Trafficspaces, will you count 10 or 100 impressions?

An ad request that results in a fallback script or the invocation of a 3rd party ad feed will not be counted as a impression on any ad campaign however because the ad request is processed by the ad server, it will be counted as a valid impression for the purpose of counting and aggregating the ad zone's total traffic stats.
